Each clinic has a consultation fee that is due at the time of your first visit. During your consultation, you will be provided with a written estimate of the charges related to the procedure(s) you are interested in. All estimates include the professional, operating room, and anesthesia expenses related to the procedure. Incidental fees such as pathology, preoperative lab work, X-ray, PMC clinic visit, and postoperative medications are separate.
All clinics require that fees be paid prior to surgery. For in office procedures all fees are due at the time of the visit.
How you pay for your healthcare will depend on whether you have health insurance and the type of coverage you have.
Some services require authorizations and referrals before you receive healthcare services. Be sure that you confirm what is required by your insurance company prior to receiving any services. You could be responsible for all charges if proper authorization or referrals are not obtained.
To ensure that registration and insurance billing is as efficient as possible for you, please provide us with complete information about your primary and/or secondary health insurance and bring your insurance card(s) with you to any visit. Inform us if your personal information has changed since your last visit.
Insurance co-payments should be paid at the time of service.
