- Turn left into Triangle Center
- Clinic is the 2nd building on the left.
From the South via I-5
- I-5 North
- Take exit 39 for WA-4 W toward Kelso
- Turn left at Allen St/WA-4W
- Continue to follow WA-4W
- Turn right at 5th Ave N
- Take the 3rd left onto Cowlitz Way/WA-4W
- Turn slight right onto Ocean Beach HighwayWA-4
- Turn left onto 15th Avenue
- Turn left into Triangle Center
- Clinic is the 2nd building on the left.
From the south via US30, Oregon
- Travel to Lewis and Clark Bridge from east or west on US30
- Enter WA-433, head Northeast to cross Columbia River via Lewis and Clark Bridge
- Continue to follow WA-433 onto Oregon Way
- Turn slight left as you approach and cross Tennant Way. Street name changes from Oregon Way to 15th Avenue
- Travel approximately 1.5 miles.
- Turn right at Triangle Shopping Center
- Clinic is the 2nd building on the left.
