• Clinic is the 2nd building on the left.
From the South via I-5
• I-5 North
• Take exit 39 for WA-4 W toward Kelso
• Turn left at Allen St/WA-4@
• Continue to follow WA-4W
• Turn right at 5th Ave N
• Take the 3rd left onto Cowlitz Way/WA-4W
• Turn slight right onto Ocean Beach HighwayWA-4
• Turn left onto 15th Avenue
• Turn left into Triangle Center
• Clinic is the 2nd building on the left.
From the south via US30, Oregon
• Travel to Lewis and Clark Bridge from east or west on US30
• Enter WA-433, head Northeast to cross Columbia River via Lewis and Clark Bridge
• Continue to follow WA-433 onto Oregon Way
• Turn slight left as you approach and cross Tennant Way. Street name changes from Oregon Way to 15th Avenue
• Travel approximately 1.5 miles.
• Turn right at Triangle Shopping Center
• Clinic is the 2nd building on the left.
