Community Hematology Oncology Survivorship Program
Survivorship is an integrated part of cancer care at our community clinics of the OHSU Knight Cancer Institute. We understand the end of your treatment is not the end of your cancer experience. Recognizing this, patients treated at our community clinics are provided with a survivorship visit in conjunction with a regularly scheduled follow up visit with their treating physician. This generally takes place about 1-3 months after completing all primary treatment such as surgery, radiation, and chemotherapy, as needed. During the survivorship visit their provider reviews the follow up plan and specific survivorship issues related to their cancer care. In addition the patient receives a personalized care plan which summarizes their cancer diagnosis information, treatment given and outlines timing of follow up visits, procedures and imaging. Additional materials are provided on issues of concern and interest to cancer survivors as well as community offerings for wellness, classes and support group programs. Our social work team provides additional individual support services.
