Declare your intent to transition to OHSU

  • Start online application – create a user ID and password if a first time user (view log-in tutorial)
  • Complete online application
  • Make sure to choose CC Enrollment Commitment option from the "Application Type" drop-down menu

Submit the following documents to our office:

1.  Official transcripts from all institutions attended
2.  Enrollment Agreement form – from the online application
3.  Non-nursing coursework plan form – from the online application
4.  Signature page – available to print once you submit your transition/application fee online
