Oregon Health & Science University is in the process of selecting a new online registration form. Until the selection is made, we are using a “work around” procedure. See the section on “Registration Form” below for additional instructions.  You are NOT automatically confirmed in the class when you complete and submit the registration form. Once we receive your completed registration form, CE staff will initiate your enrollment in your selected course modules on the Thursday following receipt of your registration form. Our courses are offered through the Sakai online platform. After you are enrolled in your selected modules, you will receive an email from “Sakai Information” with the subject line, “Pain Management Continuing Education – Welcome to OHSU Sakai”. That email will include the access information to your course modules.   A temporary certificate of completion will be sent electronically when you have completed each module. You will receive a certificate for the completion of the entire course when all modules have been completed. Although CE staff receives a periodic report of completion from Sakai and prepares the certificates, please call the CE office if you need the certificate more quickly.

Registration Fees: The fee for each module is $24.

Registration Form

Please complete the appropriate form below, depending on whether you are an OHSU employee or a member of the nursing community employed elsewhere.  Then mail or FAX the form to the CE office. The FAX number is 503-494-3691.

OHSU Employee registration form (includes OHSU hospitals & clinics, including inpatient & outpatient departments)

Non-OHSU registration form (any RN or LPN licensed by the State of Oregon or by another jurisdiction is eligible to take the course regardless of place of employment/ residence, or employment status. We also welcome other health care providers who have verified that their licensing boards will accept CE credits.

Payment Information

You can mail your completed form to the CE office with a check for payment. If you choose to use a credit card, please call our office at 503-494-2900 on Tuesdays or Thursdays, or 503-494-6772 on Monday, Wednesday and Friday to provide the credit card information. This information will be shredded after the payment is processed.  Once the financial transaction is completed, your encrypted personal information, is not stored on any server at our site. 

Your credit card will be charged as soon as the transaction is verified. The purchase will show up on your statement as OHSU School of Nursing.  In the event of unauthorized use of your credit card, you must first notify your credit card provider in accordance with its reporting rules and procedures. Under US Federal Law, your bank cannot hold you liable for more than $50.00 of fraudulent charges.
