Thank you for the submission of your proctored essay information.  The Office of Admissions will distribute the proctor instructions and essay question materials to your designated proctor within 3 business days; please be in contact with your proctor to ensure receipt of materials and to plan a date/time to sit for the essay.

Please remember, 3 copies of your essay response must be sent to the Office of Admissions. You may also print one copy for your own records.

All application materials must be postmarked by February 15, 2013. The proctored essay must be postmarked by March 1, 2013.  Late applications and essays will not be accepted.  Please plan accordingly. 

Application Status: Please check your application status at the application start page: https://sisweb.ohsu.edu/SISPRD/bwskalog.P_DispLoginNon.  Ultimately, this page will tell you what materials we have received for your application.  As this is our very busy application period, the items we receive in the mail are updated electronically as soon as we are able to data enter them.  We are unable to confirm receipt of materials; we will contact you as we process application materials if you are missing any materials and you will have time to send in the necessary items before applications go off to the Admission Committee for review.
