Students are required to provide a number of documents prior to matriculation to help the institution prepare for your arrival and clinical settings. Please see the "getting started" section of the orientation page for this information. For some cases the required materials are location specific and in those cases we have identified additional information for your review below.
CPR locations in the Portland area: All undergraduate nursing students are expected to maintain a current CPR certification. Certification for incoming Portland and Monmouth students must be at the BLS Healthcare Provider leveland must be obtained through the American Heart Association, only. Classes that include the Automated External Defibrilator (AED) are optional. For information on finding CPR courses, please contact your local chapter of the American Heart Association or check the American Heart Association CPR course finderweb page for local AHA courses.
Copy Code Form: Students may use one of the many copiers located in the School of Nursing building but will need to obtain a "copy code" in order to gain access. Use the form at the following link to request a code for your use: http://www.ohsu.edu/son/student/forms/copycardform.pdf
Oregon Nursing License Requirements: The Oregon State Board of Nursing requires all graduate nursing students and all students in the undergraduate RN to Bachelors program who will be taking clinical courses at agencies within Oregon to hold a current Oregon RN license. The School of Nursing requires that each student provide a copy of their RN license to be kept on file throughout their educational program. If you did not submit a copy of your license with your application materials, or if your license has since expired and you have renewed it, please fax a clear, readable copy of your current license to the appropriate program contact person below. When, during the course of your educational program, you renew your license, be sure to send your program contact a copy of your new license.
