Medical License

All residents and fellows must have an Oregon medical license prior to beginning training and must maintain a license throughout training.You may have either a Limited License (MD Postgraduate) or an Unlimited License (MD License).

If you are entering a fellowship, PLEASE apply for either a Limited License Post Graduate or an Unlimited License, NOT the MD Fellow License.

Limited License (Post-Graduate)

To obtain your Limited License, please go to the application page of the OMB Web site.

a. Indicate your mailing address as:

Since most incoming residents will be moving, this will ensure your license certificate is mailed to OHSU. We will make a copy of your license for our files and provide the original to you at your GME Check-In appointment. It is essential that GME has a copy of your license.

c. Indicate OHSU as your practice site – in this exact abbreviated format.

d. Indicate OHSU under training program.

e. Chronologically list all of your activities since completion of medical school.

f. If you are a PGY 2 or above, please provide payment to the Oregon Medical Board. Payment must be made online when you apply using a credit or debit card or e-check. If you are a PGY 1, GME has sent payment to the OMB on your behalf.

g. You will need to have a scanned digital passport-style photo sent electronically to the OMB. The photo should have been taken within 90 days, and does not have to be professional portrait. Email your photo to the OMB If you need, you can exit the system and re-enter at another time to complete your application, as long as you do not hit the submit button.

Unlimited License

 If you are interested in obtaining instructions and an application for an unlimited license, please review the unlimited/full license application instructions on the OMB website.

Allow at least twelve weeks for the OMB to process your unlimited license application. Residents and fellows interested in obtaining an Unlimited License effective 7/1/14 will need to have applications submitted to the OMB by early April 2014. Please be sure to track the completion of your application on the OMB web page to ensure it is issued in time for your start date.
