The 2nd Annual Culture of Safety Survey is now under way. Please encourage your staff to partake in this survey as it will assist us in addressing safety concerns.

September 21 – October 9

·         For all OHSU Healthcare staff (including physicians): Take five minutes to complete this survey about safety issues and practices in your unit. The results of this survey will help the OHSU Safety Executive Council better address existing safety concerns. All OHSU Healthcare staff members, including physicians and those without direct patient contact, are asked to take this brief survey.

·         This is our second annual Culture of Safety Survey using the Agency for Healthcare Research & Quality (AHRQ) tool. The Safety Executive Council is asking you to complete this survey because OHSU is committed to implementing initiatives to increase patient safety. This survey asks for your impressions of patient safety issues and practices in your work area.

The survey will take about 5 minutes to complete. Your honest answers to these questions will help us to address those safety concerns you have and to continuously improve safety. Our commitment is to present our results back to you in a timely manner.
