Request Credit Records
Attendance verification is available for accredited CME courses that were sponsored or jointly sponsored by OHSU School of Medicine over the past 6 years. In most cases a report will be sent to you within 10 days of receipt of your request and payment.
If you would like to obtain a transcript or duplicate credit certificate, please fill out our Credit Record Request form.
A duplicate credit memo for a single event during the current year is free. Other requests carry a fee of $20, payable by check or departmental JE.
