CME certificates for individual activities are generally printed and mailed five to six weeks after an activity has ended. CME certificates for grand rounds are issued each year at the end of the academic year.

Attendance verification is available for the accredited CME course that were sponsored or jointly sponsored by OHSU School of Medicine over the past 6 years.

Requesting a duplicate certificate for individual activity

A duplicate CME certificate for a single CME activity will be provided free of charge. Please note that there is a $20 charge if more than one certificate is requested.

Requesting duplicate CME certificates for series, such as grand rounds, M&M, tumor boards etc.

A duplicate CME certificate for a single series will be provided free of charge. Please note that there is a $20 charge if more than one certificate is requested. Transcripts for any part of the current academic year (July 1 - June 30, 2012) require manual research and carry an additional research and verification fee of $20.

Please allow 10 business days for processing your request.

Fees are payable by check made out to OHSU-CME or by OHSU departmental JE.

Information for physicians in Oregon: The Oregon Medical Board has randomly selected 10% of its licensees for CME auditing. You will be notified during the renewal process if you have been selected for a CME audit.

DO NOT send documentation to the Oregon Medical Board unless you are selected for audit.
