Referral Requirements
- Referring provider: Primary Care Physician or Neurologist
- Exceptions:
MS Clinic: Referring provider must be a neurologist
Stroke Clinic: Patients may self-refer
Wellness Clinic: Patients may self-refer - External referrals use standard OHSU Neurology referral form/process
- Pertinent medical records, radiology reports and lab results must accompany this form which can either be faxed or mailed to our office.
- Radiology films are to be on-hand at the time the patient is evaluated by our physician.
- Referrals and authorizations must be in place prior to the patient being scheduled
