Financial Policy

If you have additional questions, please call 503 494-7772
We truly appreciate that you have chosen to obtain your health care from the Department of Neurology at Oregon Health & Science University. By providing you with the following information, we hope to answer most questions you may have about our financial policies.
All patients are ultimately financially responsible for provided services. If you are required to pay for some or all of your visit at the time of service, but are unable to do so, your appointment may be rescheduled to a date when you are able to pay.

If you have insurance

For timely and accurate billing it is important that we have your most current insurance coverage information. OHSU bills your insurance and submits any unpaid balance to you for payment.

  • When scheduling your appointment, the scheduler may verify with you the type of insurance you have
  • You must be registered through OHSU registration (503 494-8505) a minimum of 2 days prior to your appointment. We reserve the right to re-schedule your appointment if this has not occurred.
  • If at any time your insurance coverage changes, please contact Outpatient Registration (503 494-8505).
  • When you check-in for your appointment, the receptionist will verify with you the type of insurance you have
  • Many insurance companies require a referral and authorization for their subscribers to receive certain types of care. It is the responsibility of the physician that refers you to Neurology or requests a follow-up visit to obtain the referral/authorization. In some instances the referral/authorization is either denied or not obtained prior to the appointment. In such instances it may be necessary to re-schedule your appointment.

Time of Service Payments

Insurance companies and OHSU require that some payments be made at the time you check-in for an appointment. Neurology will inform you of these payment requirements prior to your appointment. If you are unable to make these payments your appointment may be re-scheduled.

  • Co-payments: insurance company policy
  • Co-insurance: the percent of charges your insurance company requires their subscribers to pay
  • Unmet deductible

Non-covered services

Payment in full is required at the time of treatment for services not covered by your insurance. We will ask that you sign a Non-Covered Charges Form when you check in to acknowledge that you accept financial responsibility for charges not covered by your insurance company.

Oregon Health Plan / CareOregon, etc.

OHSU Neurology has limited appointment availability for OHP patients. Individuals covered by this plan must have valid/verified coverage prior to the appointment date and have a valid referral in place. Proof of current coverage is required at each visit, as is your co-pay (if applicable).

CAWEM coverage

Because CAWEM (Citizen/Alien-Waived Emergency Medical Coverage) does not cover non-emergent services, the section below regarding no insurance applies to those covered with CAWEM. We will ask that you sign a Non-Covered Services Form when you check in to acknowledge that you accept financial responsibility for these non-emergent services and will follow the pre-payment procedures below.

If you do not have insurance

Individuals without insurance must undergo financial screening before an appointment is scheduled. Our staff will put you in contact with this office. Once you have received a reply regarding screening, please contact us at 503 494-7772. Any sliding fee discounts granted by the financial screening office will be honored and applied after the actual charge is posted.
If you desire to forego screening and still be seen, you will be required to pay full fee at the time of your appointment.

Appointment Policy

Canceled & No Show Appointments

Failure to keep scheduled appointments or canceling with less than 24 hours notice impacts the care that we provide to all of our patients.
Neurology requires a minimum of 24 hours notice if a patient is unable to keep an appointment. This makes it possible to offer your appointment time to another patient. Please contact 503 494-7772 if you must cancel.
Appointment availability for re-scheduling late cancels or no shows are extremely limited.
If you have two (2) consecutive late cancellations or no shows, your provider may determine to discharge you from his/her clinic. If this occurs, you will be given 30 days notice during which time any prescriptions ordered by your Neurologist will be authorized for refill.
New Patients:  if you fail to cancel with more than 24 hours notice or do not show for an appointment, you will not be re-scheduled and your referring physician will be notified.

Late Appointment Check-In

If you are more than 10 minutes late for your appointment your physician may not be able to see you.
Please plan to arrive at OHSU 30 minutes early to allow for traffic/parking, check in, and completion of any required paperwork.
