Challenge
The annual OHSU Employee Giving Campaign has begun.  Once again, Family Medicine is hoping you will contribute to the FM Discretionary Account #13684. This account is a pool of funds used to advance two important causes: the professional development of our staff and the support of our most needy patients.

Professional Development Funds
Half of the total raised will be used to support staff professional development. The Allocations Committee, made up of represented/classified employees from EJH, Richmond, SWF and Scappoose, will determine how that money is disbursed. In addition:

  • Any money allocated by the Allocations Committee for staff professional development will be matched – dollar for dollar – by the Department of Family Medicine.  If Employee X is awarded $1,000 by the Allocations Committee to attend a conference, the Department also allocates an additional $1,000 to Employee X for conference expenses.

Clinic Funds
The remaining half of the total raised will be given to the clinics, based on the amount contributed by the clinic faculty and staff, for the support of needy patients.  For example:

  • If Employee X from Richmond contributes $50, $25 of that will go back to Richmond to support needy patients in a way best determined by the clinics.
  • Contributions from those who are not affiliated with a clinic will be used to support department activities including lunches at department meetings, the holiday party, resident graduation, etc.
The Clinic Challenge!

EJH and all Clinics with the largest percentage increase in donations over 2011 will earn a buffet of homemade desserts catered by Melissa De Lyser.The tentative menu includes:  Raspberry Cream Cheese Torte, Lemon Bars, Cherry Cheesecake Bites, Chocolate Layer Cake, Fudge and Toffee Bars.  Clinics that did not participate in the 2011 Employee Giving Campaign are not eligible.

The Fun Stuff

For every $10 contributed to the FM Discretionary Account #13684, employees will be given a raffle ticket. We have eight amazing prizes. For a description of each prize, click here. You can distribute your tickets among the prizes however you choose. Here's how it works:

1. Click here to print the two-page form. The Page 1 Donation Sheet is pretty straight forward. The Page 2 "Jar Sheet" allows you to divide your raffle tickets among the prizes. You must complete the Jar Sheet to have your tickets included in the raffle.

Example: Employee X contributes $100. He earns 10 tickets. He writes "5" under the 30 Minute Massage jar and "5" under the Plane Ride jar. Or he writes "10" under the Martini Madness Jar. Or he writes "1" under seven different jars and writes "3" under the Homebrew Jar. Divide them up however you choose.

2. Give the forms to your Family Medicine Employee Giving Clinic Champion or send the forms via inner office mail to: Melissa De Lyser, FM RM 317, by November 30. The prize drawing will be held on Thursday, Dec. 6, during the monthly Family Medicine Department Meeting.
