Dr. David A. Dorr, MD, MS

Dr. David Dorr

Dr. Dorr earned his BA in Economics (with minors in Mathematics and Psychology) and his MD from Washington University in St. Louis. He then completed Internal Medicine residency at Oregon Health & Science University, and earned a Master's in Medical Informatics and Health Services Administration from the University of Utah. Broadly, Dr. Dorr's interests lie in complex care management, especially for older adults and other at-risk populations, coordination of care, collaborative care, chronic disease management, quality, and the requirements of clinical information systems to support these areas. From these interests, he has broadened into clinical information needs, Electronic Health Record (EHR) deployment and Health Information Exchange as a way to expand systems-based approaches to all of health care. Finally, Dr. Dorr performs evaluations of care management and informatics initiatives using a variety of methodologies. In his spare time, he is an avid bicyclist and completed Cycle Oregon 2016, riding over 450 miles across the southern coast.

LeAnn Michaels

LeAnn Micheals OHSU

LeAnn Michaels joined the Care Management Plus team in 2023 as the Senior Research Project Manager. LeAnn holds a Bachelor’s in Food & Nutrition with a concentration in dietetics. LeAnn has been a long-time collaborator with Care Management Plus in her prior role with the Oregon Rural Practice-based Research Network, including work on projects like Nurse-based Care Management, ICCIS, TOPMED, Healthy Hearts Northwest, CPC and Meta-LARC ACP. As CMP Project Manager, LeAnn oversees projects, grants and contracts; develops proposals; and collaborates with internal and external partners. She has a passion for clinical trials and involving under-represented populations in research. When she’s not talking about research, she enjoys spending time with her family, listening to or making music, reading, hiking and card games.

Michelle Bobo

Michelle Bobo

Michelle Bobo joined the Care Management Plus team in October of 2020 as a Senior Research Assistant and Practice Facilitator. She has a M.S. in Biomedical Informatics from OHSU and a personal commitment to supporting the technology needs of health care providers. She also has a particularly strong professional interest in quality assurance and process improvement. In her spare time, Michelle enjoys a variety of interests and hobbies including crafts and sewing, gardening, and fitness.

Christie Jackson

Christie Jackson

Christie joined the Care Management Plus team in March of 2020 as a Research Associate. She holds a Bachelor’s degree in Community Health Education from Portland State University and a Master of Public Health in Global Health from Oregon State University. In her role, she provides support as a practice facilitator to Oregon primary care clinics participating in the Comprehensive Primary Care Plus initiative, a national advanced medical home model. She has also provided technical assistance to support clinics that participate in CCO incentive measure reporting in producing reliable, accurate eCQM reports using the QRDA I and III standards. Christie is interested in understanding how policy, systems, and environments (PSE) shape and constrain access to resources such as healthcare, and implementing PSE changes that make healthy choices readily available and easy accessible in the community. In her spare time, she enjoys being outdoors, playing soccer, and tending to her plants.

Gina DeNoble

Gina joined the Care Management Plus team in February of 2023 as a Research Project Coordinator. She holds a Bachelor’s of Science in Psychology from Northern Arizona University, and a Master’s of Science in Experimental Psychology from Central Washington University. Prior to her arrival at OHSU, she worked at the University of Washington managing the research portfolio of the School of Medicine’s Regional Epilepsy Center at Harborview. Here at OHSU, Gina’s time is split between Dr. Dorr’s eCarePlan project in the Department of Medical Informatics and Clinical Epidemiology (DMICE) and his FHIR Starters project within the Oregon Clinical and Translational Research Institute (OCTRI). Beyond her research background, Gina has experience in professional event coordination and community mental healthcare. When she is not working, she sings, plays guitar, and performs original music at venues around town. She has one dog named after “That Girl” Marlo Thomas.

Matthew Storer

Matthew Storer

Matthew is a software developer who manages the Care Management Plus team's technical needs, from database queries to full-stack web applications, and everything in between. Originally from Burlington, Vermont, Matt came to OHSU in early 2016 with a Bachelor of Science degree in Computer Science from the University of Vermont and 15 years of software development experience in various industries.  In his spare time, he can generally be found exploring the flow arts, juggling, performing with fire, and spinning flowersticks around Portland and at various regional festivals.

CG Brothers

CG’s primary role is providing support to Dr. Dorr and they serve as the first point of contact for the CMP Team. They hold a Bachelor’s degree in Film with a minor in English. In their role, CG serves as the team’s administrative swiss army knife, from addressing scheduling needs, to managing tech support, to handling purchase requests. CG has a wide range of experience that includes providing audio/visual support for anti-racist seminars, acting as direct in-home support for youth with developmental disabilities, and volunteering their skills as a photographer for queer foster youth. CG is interested in DEI and ensuring fair, equitable, and compassionate care for their trans siblings, as well as ensuring equitable and accessible healthcare for people from historically marginalized communities. While not at work, CG enjoys reading comics, watching movies, and hanging out with their calico.

Aisha Ghumman

Aisha Ghumman joined the Care Management Plus team in November 2022 as a Research Project Coordinator. She is a Pakistani-American and was raised by immigrant parents. She holds a Master’s in Public Health in Health Education and a Bachelor’s in Biomolecular Science from University of Michigan. In her role Aisha serves as the point of contact for COACH study collaborators, and any other study needs. Aisha is interested in research surrounding social determinants of health focusing on minorities and underserved populations. She has previously worked on research projects related to Cystic Fibrosis,  marijuana use and attitudes post legalization in Michigan, LatinX communities post Flint water crisis, and nutrition during pregnancy for low-income young mothers. In her spare time she loves to add more plants to her collection, go on walks in the nature, play tennis and baking.

Emma Young

Emma Young CMP

Emma holds a BS in Exercise Science with a minor in Psychology from The University of Mississippi, and an MS in Health Studies from the University of Memphis. Emma previously worked as a Sr. Clinical Research Assistant in the OHSU and VA Departments of Neurology with the Multiple Sclerosis centers. She managed several clinical trials for people with MS, coordinated educational symposiums, and provided support on various projects for multiple PIs. Prior to working at OHSU, she was an intern at St. Jude Children’s Research Hospital focusing on balance impairment in survivors of pediatric brain tumors. Emma is interested in research focusing on quality of life and expanding healthcare access to underserved and vulnerable populations. In her spare time, she enjoys hiking and camping with her husband, yoga, reading, and spending time with her cat, Luna.

MJ Dunne, MA

MJ Dunne

MJ Dunne joined the Care Management Plus team in August or 2020 as a Senior Research Project Manager. She holds a Master’s in Health Communication and a Bachelor’s in Social Science. In her role she manages the CMP team, serves as point person on all associated projects, grants, and contracts, and collaborates with internal and external partners to meet project deadlines and deliverables. MJ has a professional interest in achieving health equity for vulnerable populations, improving health disparities for sexual and gender minorities, critical theory, the social determinants of health, and research dissemination best practices.  In her spare time she enjoys playing with her daughter, exploring Oregon, dragon boat racing, and volunteering at the Oregon Museum of Science and Industry (OMSI). 

Taylor King

Taylor King

Taylor joined the Care Management Plus team in April of 2020 as a Research Associate and Practice Facilitator. She holds a Master’s in Public Health with a concentration in Global Health and Health Disparities, and a Bachelor’s in Human Development with a minor in Community Nutrition. In her role, she provides support as a practice facilitator to Oregon primary care clinics participating in the Comprehensive Primary Care Plus initiative, an Advanced Alternative Payment Model. Taylor has also been involved in research and grant development, as well as vendor guide development in contract work with the Oregon Health Authority. Taylor is interested in research surrounding health equity, social determinates of health, maternal and child health, and enjoys community outreach and engagement. In her spare time, she enjoys, yoga, hiking, cooking, and spending time at the coast.

Chris d'Autremont

Chris d'Autremont

Chris joined the Care Management Plus team in December of 2019 as a Senior Research Assistant. He holds a Master of Biomedical Informatics degree from Oregon Health & Science University and a Bachelor of Arts degree in Psychology from Whitman College. He provides support for the team’s work to develop a clinical decision support tool for hypertension. Chris is interested in research focused on health equity and education, mobile health, data quality, and social determinants of health. In his spare time, he enjoys skiing, kayaking, hiking, board games, and Dungeons & Dragons.

Dr. Ben Orwoll

Dr. Ben Orwoll

Dr. Orwoll is a Pediatrician who joined the OHSU faculty after completing a Pediatric Critical Care fellowship at UCSF and a Clinical Informatics fellowship at OHSU. Clinically he cares for patients in the Pediatric ICU at OHSU Doernbecher Children’s Hospital, where he also supervises medical students, pediatric residents, and critical care fellows. His clinical informatics roles include providing clinical expertise and guidance for management of the electronic health record at OHSU as well as data curation and analysis used in clinical decision support, research, and quality improvement efforts in the hospital. He works with the Oregon Health Authority to provide technical assistance and consultation around state-wide efforts to standardize and centralize the submission of electronic clinical quality measures (eCQM) among Medicaid providers. He also teaches a course on health data standards for interoperability through the Department of Medical Informatics and Clinical Epidemiology graduate program. His research efforts center around using health information technology to improve access to health data and improve standardization of care, especially for critically ill children.

Joni Toews

Joni Toews

Joni joined the Care Management Plus team in March of 2018 as an Administrative Assistant for Dr. David Dorr and the CMP team.  Joni has worked in the health care field for the past 25 years in various capacities.  Her background includes rural medical network recruiting, accounting, billing and coding and office manager.  Joni loves interacting with people and working as a team member to improve and enhance any work environment.  She enjoys spending time with her family, including her two grandchildren, playing the piano, reading and swimming.

Claire Ramey

Claire Ramey

Claire joined the Care Management Plus team in October of 2018. She holds a Bachelor’s degree in Geography from the College of Wooster. As a Senior Research Assistant, Claire has provided technical assistance to primary care clinics participating in coordinated care organization reporting and contributed to consultation services for Oregon Health Authority in regards to the development of the Clinical Quality Metrics Registry. Prior to joining the team, Claire worked as an electronic health record software tester. Claire is interested in improving the delivery and quality of mental health care, especially suicide prevention, identifying bias in data, and traumatic brain injury care. In her spare time, she enjoys cooking, camping, being a friend, and stain removal.

Dr. Christie Pizzimenti

Christi Pizzimenti

Christie joined the CMP team in 2018 as a Project Manager after earning her PhD in Neuroscience from Oregon Health & Science University in Portland, OR. Christie manages CMP’s suite of projects, including federally funded grants, collaborations with the Oregon Health Authority, and collaborations with internal and external partners, all which strive to improve the lives of Oregonians in need. Christie has interest in healthcare quality and policy, dissemination and implementation, and improving access to healthcare and healthcare outcomes in vulnerable communities. When she’s not at work, Christie enjoys camping, attending as many music festivals as possible, walking her dog, speaking French, and eating snacks.

Joshua Colasurdo

Joshua joined the Care Management Plus team in December of 2018 as a Senior Research Assistant. He holds a Bachelor’s of Science in Biology from the University of Portland. In his role, he provides support as a practice facilitator to Oregon primary care clinics participating in the Comprehensive Primary Care Plus initiative, an Advanced Alternative Payment Model. Joshua has also been involved in the research and development of information technology to address health-related social needs, as well as the Cancer Center Cessation Initiative in collaboration with the OHSU Knight Cancer Institute. Joshua is interested in broad scale primary care transformation and hopes to pursue a medical education. In his spare time, he enjoys being outdoors, particularly while surfing, skiing, and rock climbing.

Sumeet Singh

Sumeet Singh

Sumeet Singh is a research assistant for Care Management Plus and has interest in healthcare quality, health equity, and primary care practice transformation. As a Comprehensive Primary Care Plus (CPC+) practice facilitator, she provides technical assistance to Oregon primary care practices participating in CPC+, a national advanced medical home model.  Sumeet is from India, where she earned her M.B.B.S. In her spare time, she enjoys reading, gardening and spending time with her two dogs. 

Raja Cholan

Raja holds a Bachelor's degree in General Science and Community Health Studies from Portland State University, and is currently a Master's student in the Department of Medical Informatics and Clinical Epidemiology at OHSU. Prior to joining Care Management Plus (CMP) in 2013, he worked as a research assistant at various departments at OHSU including the Intensive Care Unit, Neurology, and the Center for Hematologic Malignancies. Raja has interests in healthcare quality, health care data science, and the use of Health Information Technology (HIT) to support clinical care. In his role with CMP, he has helped provide technical assistance (TA) to clinics participating in primary care transformation projects, including targeted TA around HIT and clinical quality measures (CQMs) for AHRQ's EvidenceNOW Initiative, and more general TA with overall healthcare reform on CMS' Comprehensive Primary Care Initiative. He has also helped with the deployment of 'Best Practice Alerts' in EPIC for the Clinical Decision Support study 'IQ-MAPLE' (Improving Quality by Making an Accurate Problem List in the EHR), with the implementation of select CQMs in the Integrated Care Coordination Information System (ICCIS), and with preparing the educational materials for the Office of the National Coordinator for Health Information Technology (ONC HIT) Training Curriculum Grant. Raja is originally from Portland, Oregon.

Nicholas Colin, MA

Nicholas joined the Care Management Plus team in August 2015. He holds a Masters of Arts in Biomedical Sciences from Midwestern University and Bachelor’s degree in Human Physiology from the University of Oregon. Nicholas is a Senior Research Assistant for the team, in this role he has provided technical assistance around clinical quality measures and HIT related issues for primary care clinics participating in the Healthy Hearts Northwest initiative and contributed to consultation services for Oregon Health Authority in regards to the development of the Clinical Quality Metrics Registry. Prior to joining the team, Nicholas worked as Administrator for a non-profit organization providing residential services to adults with development disabilities. Nicholas is interested in improving the delivery and quality of care for at-risk/vulnerable adult populations. In his spare time, he enjoys exploring the outdoors with the goal of seeing every national park of the United States.

Benjamin Nealy

Ben joined the Care Management Plus team full-time in early 2016 after participating in an OHSU internship program. He currently works on the Healthy Hearts Northwest project, focusing on development and management for The Pulse quality measures performance application. Ben is a graduate of Brigham Young University with a Bachelor's in Statistics/Biostatistics, and a current Master of Science in Public Health in Biostatistics candidate at Emory University.

Doug Rhoton

Doug, a Senior Systems Architect, joined the Care Management Plus team as a contractor in 2008, moving into a full OHSU employee role in 2010. His expertise covers all things data-related, and he supports the team by developing specialized tools and applications for projects such as Care Management Plus, the Integrated Care Coordination Information System (ICCIS), the Knight Cardiovascular Institute (KCVI), the Commonwealth Fund, and Healthy Hearts Northwest. Doug earned his technical degree in the 1980s, lives at the Oregon Coast, and enjoys fishing, engineering, repairing, and building.

Rachel Ross

Rachel joined the Care Management Plus Team in 2016, providing support as a Regional Learning Faculty member for the Comprehensive Primary Care initiative. She also coordinates various aspects of a research study focusing on risk stratification implementation in primary care. Previously, her experience in neurology centered around cognitive decline prevention in older adults. Rachel studied psychology and neuroscience at Willamette University, and spends her free time taking ballet classes and expanding her knowledge of cetacean species.

Bhavaya Sachdeva, MPH

Bhavaya joined Care Management Plus in February 2015. She holds a Master of Public Health degree from OHSU and a Bachelor's degree in Chemistry and Business Administration from the University of Oregon. She is the Program Manager for the team, providing leadership and oversight to all grants and projects, with a more involved role in the CPC initiative, H2N, Improving Implementation of Risk Prediction in Primary Care, and TOPMED. She is responsible for strategic planning, collaborating with external research partners, managing the team, and overseeing operations.

She previously worked as a Project Manager in the Department of Family Medicine at OHSU. She managed operations for the Data Team, and coordinated multiple grants related to using data for quality improvement and health delivery, preparing residents to care for the mental health needs of patients, and improving documentation for depression in the EHR. Bhavaya also has previous clinical experience in an Urgent Care setting, as well as with quality improvement and Medical Home transition projects at a CHC. She is passionate about pediatric global health and working with underprivileged youth. Bhavaya grew up in Eugene, Oregon and enjoys traveling, trying new vegetarian cuisine, hiking, cooking, and dessert and wine tasting.

Jesse Wagner, MA

Jesse is a research associate who joined Care Management Plus in February 2013.  He has a Master’s degree in Sociology from Portland State University and a Bachelor’s degree in Sociology and German from Virginia Commonwealth University.  Jesse works on a number of projects for the team, including CPC, Risk Prediction in Primary Care, TOPMED and previously on the Multiple Chronic Conditions study. Jesse has an interest in making data actionable, primary care practice transformation, socio-cultural dynamics, and mixed-methods research. Jesse previously worked at the Northwest Evidence-based Practice Center where he worked on systematic reviews and identifying future research needs for different topics.  In his free time, Jesse enjoys soccer, seeing live music, hiking and camping, and hanging with his dogs.
