The following credit cards are accepted for online registration:  VISA, MasterCard, American Express, and Discover.

If you choose to register via our secure server, all of your personal credit card information will be encrypted before it is transmitted over the internet. (Note the change from http:// to https:// in the URL as you enter our security zone.) This means that data cannot be read as it travels through the internet to our system. Our secure server, certified by Verisign, provides the highest level of security available. The charge to your credit card will appear on your statement as "PAYPAL *OHSUDCHFOUN".

2. Complete the registration form. If paying by credit card, fax form to 503-494-1409.

3. Mail check (made out to OHSU Foundation) and completed registration form by July 31 to:

Refund Policy

We will accept cancellations until July 31, 2014. All cancellation requests must be in writing, and must include your home address and social security number (if original method of payment was via check), in order to process your refund. No refunds will be given for cancellations received after July 31, 2014.

Cancellations received by July 31, 2014 will receive a refund, less a 25% processing fee, and will take 2 to 3 weeks to process depending upon the method of payment. Should you wish to substitute a colleague, simply fax or email notification of their name and contact information by August 5 so that we may prepare registration materials.
