Request OHSU Library Materials
Use this form only if you do not have an OHSU Library barcode.
If you have an OHSU Library barcode, use the “Request” function in the catalog.
To apply for an OHSU Library barcode online, please use the library barcode application.
If you do not have an OHSU Library barcode, use this form to request books or journal volumes from the OHSU Old Library or off-site storage facility. Up to 10 requests will be processed per day.
- Check the catalog to verify that requested items are owned by the library.
- Materials will be delivered to your selected location for copying or pickup.
- You will receive a response only if your request cannot be filled.
- The Library also provides a fee-based Photocopy Service.
To schedule an appointment for browsing or research, please e-mail email@example.com. Appointments will be scheduled within 3 business days.
Paging ServiceIf you need more items pulled than the daily limit (10), library staff will pull materials and make them available for your use in the Main Library, Science & Engineering Library or for checkout with an OHSU Library barcode. Materials will be pulled on a variable schedule within 2 business days and you will be notified when your materials are ready. There is a $20/hour fee for this service. Minimum charge is $10.

Thank you for submitting your request. Items will be held at the circulation desk at your selected location for 1 week or delivered to your campus mail address. Please return materials to an OHSU Library or via campus mail (OHSU Library-LIB).
