Web Database Request

ITG's procedures now require all new database requests to be made through iSTART. In addition, a separate database request form must be completed and sent to itgdba@ohsu.edu. Not only do these new procedures help us to gather the necessary information to assess your needs, but they also help us to more accurately track projects, maintain regulatory compliance, and balance DBA resources. Once you have submitted the iSTART request and returned the completed database request form, we will then acquire the appropriate required approvals and get started on your project.
