How to Complete Your Gift
An easy way to include a gift in your will or trust is to state the specific dollar amount you'd like OHSU Foundation to receive. Another option is to designate that we receive a certain percentage of the residue of your estate.
You will feel secure knowing that your assets are always there for you should you need them. Most important, it can help fund your desire to be part of our future success. Your gift of cash can be completed in four steps.
1. Determine how much you would like to give. You can choose a certain dollar amount or you can leave a percentage of the residue of your estate, so that your gift remains proportionate to other bequests.
2. Decide how you would like your cash gift used. Do you have a particular program in mind that you would like to fund with your gift? Will your gift be unrestricted, enabling us to use your gift for our most pressing needs? If you choose to specify the ultimate purpose of your gift, please let us know so we can ensure that your intentions can be fulfilled.

3. Meet with your estate planning attorney.
Discuss your possible tax and other benefits with your attorney to ensure that this bequest meets your goals. An estate planning attorney can prepare your will or trust if you do not have one, or prepare a codicil to your will or an amendment to an existing trust.

It is important that the bequest designation read "OHSU Foundation." The foundation has been established to receive, manage, and disperse funds in the best way possible for the benefit of OHSU.
Basic Sample Bequest Language
"I hereby give, devise and bequeath ________ (a specific dollar amount or percentage of the residue of my estate) to the Oregon Health & Science University Foundation, a non-profit corporation of the State of Oregon, to be used to further the purposes of the Oregon Health & Science University in the area of greatest need."

Beginning July 1, 2011, a 2.5% administrative fee may be applied to gifts.
During your lifetime, you can change any of the provisions in your will or trust. The will or trust, along with any changes, should be prepared by a qualified estate planning attorney in your state. Each state has unique rules for proper execution of these documents.
