Certifications and Insurance

Compliance is a very important part of being a student. It is the student's responsibility to stay current in all compliance areas. Please note HIPAA training and Background Checks must be completed as soon as possible and no later than the date posted on the checklist to allow OHSU School of Nursing the ability to communicate and to allow access to appropriate systems for students.

Please return the following forms to:

HIPAA and Respect at the University
As part of the HIPAA privacy standards, OHSU is required to educate all members of its workforce on how HIPAA will affect their role at OHSU. OHSU's workforce includes all employees, students, faculty, staff, and volunteers. OHSU has developed a computer based education program called “Privacy and You.” All students are required to complete the tutorial prior to the start of classes and return a copy of their certificate to the Office of Admissions.

Respect at the University, another University required training module, is also available online. Please visit http://www.ohsu.edu/cc/ed/bb/ to complete HIPAA and Respect at the University.

If you are a new student, please choose Get Started | Register Here, then choose New OHSU Student - student ID not assigned. The trainings will require approximately two hours total to complete. You may save and continue later as necessary.

Background Checks
All admitted students must complete and return a completed background check. For more information on the background check policy, click here.
Drug Screening
Effective July 1, 2008: New students enrolling in programs with clinical placements will be required to successfully pass a drug screen prior to starting classes. This will include students in all of the School of Nursing programs except the PhD and MPH programs. Details about the protocol for this test will be sent to you and will be posted on the School of Nursing web page when they are finalized. Some clinical placements will require additional drug testing immediately prior to that placement. Additionally, students may be tested for alcohol or drugs on a for-cause basis. Please contact the Office of Admissions with any questions at proginfo@ohsu.edu or at 503 494-7725.

CPR
All students must show evidence of current certification in CPR training for Health Care Providers (includes infant, child and adult). This regulation is required by all clinical agencies. For information regarding CPR training classes required for your specific campus, please check your specific campus site linked below for details.  Note: Incoming students to the Portland campus must take a health provider CPR course through the American Heart Association.

Finding a CPR course can sometimes take up to six weeks, so we recommend students begin looking for a course now. We recommend that students look for a course that has a 2 year expiration date v ersus a one year expiration date.

If you apply for the waiver because you have a qualifying group insurance plan be sure to include a copy of the schedule of benefits of that plan (you can get this from the insurance company) and submit a copy of your insurance verification card with expiration date highlighted along with the OHSU Waiver Application form. Insurance waiver requests are subject to approval by the Student Health Center. Note: Students in the RN to Bachelors Degree Program for nurses should contact the RNBS Program regarding this requirement.
