Required
Immunizations
Students enrolled
in the OHSU School of Nursing program may be exposed to communicable diseases
during clinical rotations resulting in manifestation of the disease and
possible spread to patients and other co-workers. In order to protect
students, other health care providers and patients, OHSU requires students
to provide evidence of adequate immunity to Hepatitis B, Varicella (Chicken
Pox), Mumps, Tetanus Diphtheria, MMR and PPD.
The School of
Nursing Immunizations Policy: For the protection of the public health
and benefit of students, requires that students as a condition of enrollment
must present appropriate proof of a current tuberculin skin test, chest
x-ray (if positive tuberculin) and proof of up-to-date immunizations
for: diphtheria-tetanus; mumps; rubella; rubeola; hepatitis
B or proof of prior disease. Immunization for polio is recommended for
all students but is not required. Students who are
unable to meet these requirements due to medical conditions shall furnish
an explanation from the health care provider which the Associate Dean
and/or director of student health will review who will then make a recommendation
to the Vice President for Academic Affairs for a waiver if appropriate.
Students not meeting the immunization requirements may be prevented
from engaging in the clinical or practicum portion of their program.

Bloodborne
Pathogen Training
Federal law mandates that all individuals at risk for exposure to bloodbome
pathogens receive training annually about how to protect themselves. The
School of Nursing has arranged training for students during orientation.
If are an RNBS or
graduate new student and you have received Bloodborne Pathogen Training
within the last year, please submit a Bloodborne
Pathogen Employer Verification form. If you do not anticipate any
patient contact you may request and Exemption
the exemption normally applies to Doctoral students.
Immunization
Students need to complete
the Immunization Forms
and return it to the contact person noted on the form of your program
or campus by the date indicated on your checklist. Please note, students
failure to provide complete documentation will result in an unexcused
absence from the clinical setting. Students will be allowed to participate
in clinicals only after the immunization criteria has been met.
