OHSU School of Nursing Background Check Instructions
All admitted students must submit their state of residence or FBI criminal background check (CBC) and finger print report (FPR) upon enrolling (view
policy) to the School of Nursing. Students may be prevented from participation in clinical experience without this documentation.
Step 1 – Get Fingerprinted
This request for criminal background check needs to contain a fingerprint card. You will need to bring an official picture ID card with you to get fingerprinted; your student ID is not acceptable. This can be a driver’s license, state ID card, passport, or military ID.
There are several places to get fingerprinted.
1. You can look in the phonebook under fingerprinting to find other companies that provide this service in your area.
2. You can go to your local sheriff’s office. Most places ask that you call ahead for an appointment.

3. The Oregon State Police also offers fingerprinting service; call 503-378-3070 for information. They can also handle your criminal background check directly.
4. In some communities, the local police station can do your fingerprints.
Helpful Hint for Fingerprinting Nurses:
5. Apparently nurses can be hard to fingerprint due to the frequent hand washing and wearing of gloves. Hydration is key to good prints. If you can begin using hand lotion for a week before printing, it will be helpful.
Step 2 – Request a Criminal Background Check
*If you have lived in Oregon for the past three years:
- The Oregon State Police website is very helpful

- Send a letter, which includes your address, along with your fingerprint card and a check or money order for $33 made payable to the Oregon State Police, to the Oregon State Police stating that you want a copy of your criminal history record.

*If you have lived in more than one state during the last three years, you must submit a national FBI check in lieu of a State background check:
- Write a letter to the Federal Bureau of Investigation asking them for a record of ID for your personal use. List your return address, your current and any former names, and your date of birth. Include a certified check or money order payable to the U.S. Treasury for $18, along with a fingerprint card.

Step 3. Submit the Forms to OHSU
- The State Police or the FBI will return your fingerprint card and send you your criminal history report. The State Police will put the report in the form of a letter. The FBI will stamp it on the back of your fingerprint card.
- The School of Nursing needs ONLY a copy of the report. Photocopy the letter from the State Police or the front and back of the stamped fingerprint card from the FBI and send to the address below. Keep the original fingerprint card and report for your files.
