  Note: If you are a continuing or new student with a current 2008-2009 insurance waiver and plan on continuing to waive the insurance again during the 2009-2010 school year, your waiver deadline is the last day of the month prior to the month in which your fall term classes start. For example: if your fall term begins with any day in August you must turn in your waiver prior to the end of July. For a September start date it must be in by the end of August. This deadline is always the month prior to your fall term no matter the month your fall term begins. You MUST turn in a waiver each year according to the aforemention schedule even if your coverage has not changed.

  Attention: If you are a new student beginning your program in June or July you may sumbit the 2009-2010 waiver, but you must indicate on the waiver your program and start date, and the fact you would like this waiver to also cover June and July in addition to the entire 2009-2010 school year.
