Transcripts

You are required to submit an official transcript from the institution where you completed your master's degree in nursing.

This is a requirement of OHSU. An institution's transcript shows official credits only for credits taken at that institution. It is not an official transcript for transferred credits.

Note: If you are applying to the PMCO option with a non-nursing master's degree you should submit your master's transcripts as well as your most recent nursing degree transcripts.

Request that the transcripts be mailed to you and send them in their sealed envelopes with your application.

For our purposes, transcripts are official only if the seal is not broken.

If your application does not contain an official transcript from each institution, the admission decision made about your application will be delayed. However, if you believe obtaining an official transcript may delay your application you may submit an unofficial copy with your application materials as long as you are in the process of obtaining official transcripts. If you have taken courses at OHSU, contact the Office of the Registrar 503-494-7800 to have your OHSU transcript placed in your application file.
