Your completed application should include:

  • Application materials including final signed page from online application
  • Application fee
  • Transcripts
  • Letters of recommendation/reference
  • Professional resume' or curriculum vitae
  • Test scores sent directly from testing agency

Please put your application materials in a packet format, but do not staple or bind your application in any way. Send completed application packets to:

Applications must be accompanied with a $120 applications fee (check or money order) made payable to OHSU or the application will not be processed.
