Preparing to Apply

The OHSU School of Nursing has an online application process.

The application is simple to use and allows applicants will be able to track their application progress. You will be able to see when items you've submitted, such as transcripts, have been received by the Office of Admissions and when a decision has been made about your application status.

Please gather and/or prepare the following materials for submitting your application, click on each link for detailed information. Items outlined below should be included in your application packet.

Application Deadlines: Fall Term 2008: Priority application deadline is December 1, 2007.
Priority is given to those applications received on or before the application deadline date for admissions and scholarships . Applications received after December 1 will be reviewed if space is available.

Transcripts: You are required to submit an official transcript from each school you have attended after high school. An institution's transcript shows official credits taken at that institution. It is not an official transcript for transferred credits.

Request that the transcripts be mailed to you and include them in their sealed envelopes with your application. Transcripts are official only if the seal is not broken.

If your application does not contain an official transcript from each institution, the admission decision made about your application will be delayed. However, if you believe obtaining an official transcript may delay your application you may submit an unofficial copy with your application materials as long as you are in the process of obtaining official transcripts. If you have taken courses at OHSU, contact the Office of the Registrar 503-494-7800 to have your OHSU transcript placed in your application file.

Scholarly Writing Example

Please submit one document that best exemplifies your scholarly writing where you are the first or single author that demonstrates expository and analytic skills; such as, published articles, technical reports, manuscripts, project reports and class papers.

Letters of Recommendation (References): Three letters of recommendation submitted in sealed envelopes from people who can speak to your academic skills and potential for success in a research focused Ph.D. program. Ask that they identify how they know you and your work.

We request that the letters be returned to you in sealed envelopes and send the references with your application. For our purposes, reference letters are acceptable only if the seal on the envelope is not broken.

Graduate Record Exam (GRE):

  • Plan to take the GRE at least 4-6 weeks prior to the application deadline.
  • A minimum combined score of 1000 (verbal and quantitive) is expected.
  • An official copy of your Graduate Record Examination (GRE) scores.
  • The scores must be sent directly from the Educational Testing Service to the Registrar's Office and upon arrival will be matched with your application. As a precautionary measure, you should request an official copy of your GRE score for your files.
  • Please contact ETS at (510) 654-1200 or by logging on to the GRE Web site, for registration, test dates and testing locations.
  • The GRE code for OHSU School of Nursing is 4900.
  • If you have taken GRE's and have an official copy please include it with your application.
  • If you have taken the GRE's and have a "copy" of your official GRE scores please include the copy with your application along with the GRE WAIVER FORM.

Statistics: Ph.D. students must have successfully passed a statistics course with a grade of "B" or better within five years prior to the first quarter of the program, whether the student is enrolled part or full time. The statistics course will not count toward the degree requirements in the student's program of study. A statistics course which includes inferential statistics is highly recommended for Ph.D. students.

Apply to the Ph.D. Program

To complete your application to the OHSU School of Nursing Ph.D program, you will need to complete an online application, submit a $120 application fee, and submit supplemental application materials to our Admissions Office by the December 1st priority application deadline. All supplemental materials need to be mailed together as a packet. Caution: If you do not submit all application materials and the application fee by the priority application deadline, your application and subsequent review could be significantly delayed.

  • Application materials including final signed page from online application
  • Application fee
  • Official Transcripts
  • Copy of RN license or eligible for licensure in Oregon
  • Letters of recommendation/reference
  • Professional resume' or curriculum vitae
  • Example of scholarly work (in English)
