Sending your application materials for the Online Master of Public Health:

Your completed application should include:

  • Application fee
  • Transcripts
  • Letters of recommendation/reference
  • Professional resume' or curriculum vitae
  • Test scores sent directly from testing agency

Caution: If you do not submit all application materials in one envelope, your application and subsequent review will be significantly delayed.
Applications must be accompanied with a $120 applications fee (check or money order) made payable to OHSU or the application will not be processed.
