This section of the GME webpage will take you though a process which will produce a printable Program Letter of Agreement for you. These forms must be used any time a resident/fellow does any training off-campus. They must be fully signed before any off-campus activities begin or the program will be responsible for funding the salary and OPE of the residents/fellows until the time at which they are fully signed.

First, the Program Director and/or the resident/fellow must evaluate the educational value of the rotation or elective. Then the Program Coordinator or person creating this form will go through a series of questions and answers. Please note that all of the questions must be answered.
