Family Medicine / Preventive Medicine

Practicum Overview

The purpose of the practicum is to give the resident experience in practicing general preventive medicine and public health in established sites under the guidance of skilled practitioners. Residents in the combined family medicine & preventive medicine track will be exposed to a broad range of experiences in a variety of public health and preventive medicine settings. They will also have the opportunity to spend concentrated time in an area of their particular interest.

The practicum experience is mostly concentrated in the 4th year of training. During the MPH months in the spring of the third year, however, the resident spends one half-day per week attending preventive medicine seminars at the Department of Public Health and Preventive Medicine. During this stretch, the resident will explore possibilities for where and how to spend their 4th year practicum experience.

In the fourth year, the resident continues to attend preventive medicine seminars band devotes three to four days per week to the practicum experience(s). The rest of the time is devoted to completing MPH coursework.

Practicum Experiences

The resident will have several experiences to choose from:
  • Associate Medical Director in managed care administration at CareOregon, the largest and principal payer for medicaid services in Oregon.
  • Research at Kaiser Permanente Center for Health Research
  • Local public health experience on a range of public health needs in a large metro area health department or in a rural but close by health department.
  • Communicable disease control at the Oregon Department of Human Services
  • Maternal-child health at the Oregon Department of Human Services
  • Environmental health at the Oregon Department of Human Services

Managed Care Administration

If the resident is interested in developing population-based management skills, the resident will serve as Associate Medical Director of Care Oregon, a Medicaid managed care organization. David Labby, MD, Care Oregon Medical Director and faculty member for the FM/PM track, will supervise the resident. The resident will apply population-based methodology with Dr. Labby in assessing the health needs of the plan enrolled population, developing health plan policy, developing clinical guidelines, reviewing and assessing health care utilization and access and assuring quality of care given to plan members. The resident will gain additional experience in budgeting, fiscal control, personnel practice and administrative techniques, including automated record keeping and computer applications in the management of health services.

Research

Residents interested in focusing on research can spend their practicum experience at the Kaiser Center for Health Research. Specific research projects could include: evaluation of managed care and other alternative approaches to organizing, financing and reimbursing health care services; practice patterns of physicians and other providers; risk adjustment for population-based reimbursement systems; cost-effectiveness and cost-benefit analyses; medical treatment outcomes; clinical practice guidelines/shared decision making; and dissemination and assimilation of health and clinical information by providers and patients.

State Public Health

Residents choosing their practicum at the Oregon Department of Human services will have a choice of experiences in several state programs, such as maternal-child health, communicable disease control, chronic disease management or environmental health. As part of their experience, they will have an opportunity to take call for responding to communicable disease outbreaks throughout the state under the guidance of health division epidemiology staff.

Local Public Health

Residents will have an opportunity to shadow the residency director in order to pursue interests in public health projects to include air quality, maternal child health, needs assessments, emergency planning and communicable disease response. As a deputy local health officer, the residents will gain experience rich in preparing for a career in local public health.
