OPTH 631 - ORAL & MAXILLOFACIAL PATHOLOGY

Case #1:

52 year old man presented with: a 1.0 x 0.7 cm multilocular radiolucency between teeth #s 22 & 23.

A reasonable radiographic differential Dx:

• Ameloblastoma
• Lateral periodontal cyst (botryoid variant) (this is what it was)
• Central giant cell granuloma

Case #2:

48 year old man with: a 6 x 4 x 3 cm normal colored subcutaneous swelling anterior to the sternocleidomastoid muscle below the left angle of the mandible.

A reasonable clinical differential Dx:

• Lymphadenopathy (neoplastic or inflammatory)
• Salivary gland neoplasm
• Benign cervical lymphoepithelial cyst (this is what it was)

Case #3:

60 year old woman with: a patchy leukoplakia involving the entire soft palate.

A reasonable clinical Dx:

• Candidiasis (it was ­ see cytology smear with fungal hyphae)
• Hyperkeratosis, r/o dysplasia
• Chemical burn

Case #4:

This 35 year old woman has: A patchy red eroded and brown lesions on the anterior hard palate in the incisor and bicuspid area.

A reasonable clinical differential Dx:

• Pizza burn (this is what it was)
• Candidiasis
• Frictional trauma

Case #5:

This 48 year old man presented for dental treatment: He was noted to have generalized yellow discoloration of the skin.

A reasonable clinical Dx:

• Jaundice
• Hepatitis
• Obstructive jaundice
• Carotemia (he was a carrot juice addict)

Case #6:

This 40 year old man has: Generalized red hyperplastic attached gingiva. Also there is generalized white accretions on all of the teeth.

A reasonable clinical differential Dx:

• For the gingiva:
  • Hyperplastic gingivitis (this is what it was)
  • Scurvy
  • Wegner's granulomatosis
• For the teeth:
  • Gross calculus (this is what it was)
  • Hypocalcified amelogenesis imperfecta
