Education and Background:

David Cruikshank earned his D.D.S. from the University of Maryland and his Certificate of Orthodontics from Oregon Health & Science University.

Teaching & Professional Affiliations:

Dr. Cruikshank is a clinical assistant professor with the OHSU Department of Orthodontics who has been in private practice since 1970. His subspecialty is in Practice Management. He maintains memberships with the American Association of Orthodontists, Pacific Coast Society of Orthodontists, Oregon State Society of Orthodontists, Angle Society, and the American Dental Association.
