OHSU
Advanced Specialty Education Programs 2008-2009
The School of
Dentistry offers five advanced specialty education programs in Endodontology,
Orthodontics (M.S.), Oral
and Maxillofacial Surgery (M.D.), Pediatric Dentistry and Periodontology.
Admission to a postgraduate program leading to a Certificate or
combined Certificate/Masters is granted by the Advanced Education
Committee.
General
Application Procedures
All applicants are required to submit an application through the Postdoctoral
Application Support Service (PASS). PASS is a centralized application
service for more than 460 participating postdoctoral dental education
programs. All PASS application materials may be requested online
at www.adea.org. Please follow
PASS application instructions with regard to the submission of
application materials such as transcripts and general letters of
evaluation/recommendation.
Please refer to the chart below for application requirements specific
to each program.

Supplemental Application/Processing Fee (Endodontology, Oral and
Maxillofacial Surgery, Orthodontics, Pediatric Dentistry and Periodontology)
In addition to a complete PASS application you are also required
to complete the Oregon
Health & Science University School of Dentistry Application for Admission To Advanced Specialty Education Programs. A $75.00 non-refundable application processing fee must accompany your application. Please send your completed application and a check or money order made payable to OHSU in the amount of $75.00 to:

Dental
Match
OHSU’s Oral and Maxillofacial Surgery and Orthodontic programs
participate in the Postdoctoral Dental Matching Program. If you
are submitting an application to either of these programs you must
register to obtain a Match number. Please include your Match number
on your PASS application. To obtain a Match number please visit
www.natmatch.com/dentres/.
Graduate
Record Exam (GRE)
Applicants applying to our Masters/Certificate Program in Orthodontics
are required to take the General Test of the GRE. The test is composed
of analytical writing, verbal, and quantitative sections. For general

Official
Transcripts and Letters of Recommendation/Evaluation
All application materials such a official transcripts and
letters of recommendation/evaluation must be submitted to PASS.
ADA
National Board Examinations Part I and Part II
All students completing their dental education in the United States
should have completed the ADA National Boards Part I and II prior
to graduation and licensure. Most of our programs require Part I
scores at the time of application and Part II scores prior to matriculation.
If you are a graduate of a foreign dental school, although not required,
it is highly recommended that you complete both.
Test
of English Foreign Language (TOEFL)
TOEFL is required for those students whose primary language is not
English. Applicants should achieve a minimum TOEFL score of 250
on the computer-based exam, 600 on the paper-based exam or 100 on the internet-based exam. For more

Financial
Aid
OHSU is committed to providing access to academic programs in an
environment that stimulates the spirit of inquiry, initiative and
cooperation and is similarly committed to helping student’s
finance their education. Over 90% of students enrolled at the School
of Dentistry receive some type of financial assistance. Both federal
and private loans are available to US citizens and permanent residents
of the United States. Financial aid applications are available in
January/February each year on the OHSU Financial Aid website. Detailed
information on financial aid can be obtained from the Office of
Financial Aid at www.ohsu.edu/finaid/.
Residency
Requirements:
In Oregon, as in other states, instruction fees at publicly supported
four-year colleges and universities are higher for nonresident students
than for resident students. Currently, nonresident students are
assessed instruction fees that approximate the full cost of instruction.
The current policy used in determining residency seeks to ensure
that only Oregon residents are assessed the resident fee. Detailed information regarding OHSU's Residency Policy can be obtained from the University Registrar at (503) 494-7800.
Reapplication
Applicants who have submitted applications in previous years and are seeking
to reapply to an advanced specialty education program must submit
an entirely new application and supporting documentation. No materials
from previous applications will be used to complete your application
documentation requirements.
Deferred Admissions
OHSU does not offer an option to defer admissions. Accepted applicants who wish to postpone enrollment must reapply through PASS during a subsequent application cycle.
Matriculation
and Orientation
It is the responsibility of all admitted students to complete their
admission files. A final set of official transcripts from all post-secondary
schools (undergraduate and dental schools), official ADA National
Dental Boards, Part’s I and II (if applicable) , and official
GRE (orthodontic only) scores will be requested prior to matriculation.
Not receiving this documentation in a timely manner will result
in a hold being placed on registration privileges, financial aid,
and transcripts. The Office of Admissions and Student Affairs will
request this documentation prior to matriculation.
Foreign
Dental School Graduates
International dental school graduates are eligible to apply to OHSU’s
Orthodontic and Pediatric Dentistry programs for the
class entering in 2008. In addition to the requirements listed above,
applicants should also provide official TOEFL scores, official transcripts
in the original language and certified English translations, an
ECE Credential Evaluation (course-by-course evaluation), and certified
copies of dental diploma and license to practice dentistry in the
original language and certified English translation. Supplemental
documentation should be mailed directly to the Office of Admissions
and Student Affairs.
Beginning with the 2008-2009 application cycle, applicants applying to our advanced specialty education programs (Endo, Ortho, OMFS, Pedo, and Perio) must be graduates of a CODA (Commission on Dental Accreditation) approved dental school. Applicants with foreign dental degrees will no longer be eligible for admission to our advanced specialty education programs.

Student Visas
OHSU will provide an I-20 only to those students that gain admission
to one of our advanced specialty education programs. To acquire
an I-20 you will be asked to provide a financial statement from
a bank savings account that documents enough funds to cover the
total cost of all educational and living expenses for one year.
Health Requirements

Criminal History Reviews
Admitted applicants must complete a criminal offender inquiry and undergo a criminal history review as part of the matriculation process. Prior conviction does not necessarily disqualify an admitted applicant from matriculation. The admitted applicant has the right to inspect and challenge the results of the inquiry. Information about federal civil rights law and arrest records may be obtained through the Bureau of Labor and Industries.
Drug Screening
Beginning in 2008, all newly admitted students must successfully complete a drug screening within 14 days from the date of matriculation. This requirement is one way OHSU demonstrates its commitment to maintaining a safe and healthy environment for employees, students, patients and visitors. Positive test results may immediately disqualify an admitted student from enrollment. The admitted student has the right to inspect and challenge the results of the inquiry.
OHSU policy indicates that no person shall be under the influence of any unlawful drug, any alcohol, or any misused prescription drug while on OHSU premises. Students suspected of being under the influence in violation of this prohibition may be required to submit to a drug or alcohol test at cause.
