Required Documentation for Outgoing Subcontracts (Needed at Time of Application Review)
*This list is subject to change based on updated electronic submission procedures.
The following documents are needed from each of the consortium/subcontractor site(s):
- Subcontracting Institution Certification
- For NIH Applications:
- Signed PHS Face Page from subcontractor organization (Preferred)
-OR-
- Statement of Intent or Willing Letter from the subcontractor organization indicating their willingness to
collaborate on the project, signed by their authorized Institutional Official; include the required NIH/PHS
wording and all information indicated in the
EXAMPLE
- For non-NIH Applications:
- Statement of Intent or Willing Letter from the subcontractor organization indicating their willingness to
collaborate on the project, signed by their authorized Institutional Official; include the applicable agency or
federal language, and all information indicated in the
EXAMPLE
- Budget (on appropriate agency forms, if applicable)
- Detailed budget for initial year budget,
- Summary budget for entire project period budget, and
- Budget Justification
- Scope of Work (recommended at application, required at time of award)
- Biosketches for all Key Personnel
- NIH Checklist (if applicable)
- Resources Page (optional)
- Other Support (if applicable)
Incoming subawards/subcontracts are handled by RGC in a manner similar
to standard applications. Requirements vary based on the sponsor and the type of contract.
Please contact your GCA for specific information. In most cases, the following documents
should be provided to RGC at the time of application review:
- Application Packet
- Budget (for OHSU)
- Scope of Work (for OHSU)
- Signed PPQ
