Access is restricted to personnel who have a demonstrated need to be in the facility,
are enrolled in the OHSU Occupational
Health and Safety Program, are listed as participants on an approved Institutional Animal
Care and Use Committee animal protocol
and who have undergone appropriate training.
- The Principle Investigator must contact Dr. Barbara Cox, IACUC Manager at coxba@ohsu.edu to request your addition to the animal protocol.
Please retain a copy of this email to expedite your access request.
- You must be enrolled in the OHSU Occupational Health and Safety Program. Please complete the Animal Participant Review & Health Questionnaire
- Complete the Comparative Medicine Access Questionnaire
- Complete the OHSU Access & ID Card Request Form and/or
key request form and have them signed by your Department Manager. Failure to provide a signed form will
delay your ability to get access.
- Some areas may require additional training before access will be granted. Contact the Department of Comparative Medicine for more information.
- Bring the competed forms in 1-4 above to the Department of Comparative Medicine Offices located in the Biomedical Research Building B110 for final signatures.

All persons entering the animal facilities are required to have a visible
OHSU Photo Identification badge per OHSU policy No. 07-09-001. http://ozone.ohsu.edu/policy/pac/chapt_7/7-90-001.htm
You must also follow the OHSU Department of Public Safety's Identification Card/Access Card Guidelines
http://ozone.ohsu.edu/pubsafety/id/id_guidlines.pdf which include never loaning your badge to another person and never
allowing someone to "tailgate" into the animal facility.
Suspicious persons or activities should be reported to Public Safety 4-4444.
Photography of any kind is strictly prohibited without the written permission of DCM Management.
All OHSU staff members, visitors*, and volunteers must be included in the
Occupational Health and Safety program. Everyone must complete the Animal
Participant Review & Health Questionnaire before entering the animal area. Allergies to laboratory animals affect more than 30 percent of animal
users. The use of face masks, gloves, and clean laboratory clothing, as well as frequent hand washing and avoiding direct animal contact, are the
best methods to prevent such allergic responses. Persons entering animal housing rooms or working with animals must comply with the OHSU IACUC Policy
on Requirements for Protective Clothing and Identification While Working
on the Markham Hill Campus
Laboratory animal users must inform Comparative Medicine management about incidents of cuts, punctures, and animal bites, especially
those that involve non-human primates.
