NOTES:	Incidental, Technology, Building and Health Service Fees are at OIT rates except that the Health Service Fee includes $3 per term for coverage against needle stick accidents.
	Students are required to purchase OHSU major medical insurance or a policy approved by OHSU.  There is an OHSU major medical insurance waiver option available.

	Major Medical/Vision/Dental Insurance is based on the length of the term (monthly rate X number of months in term).  The above rate reflects a 3 month term.  Fall Term rates are:
	major medical = $1,047.12, dental = $91.40, vision = $31.28.
	Lab fees are assessed for specific masters-level courses, in addition to the fees shown above. Students eligible for the OHSU employee tuition benefit are required to pay lab fees.
	A fee of $40 per credit is assessed for all masters research/practice improvement project and doctoral dissertation credits, in addition to the fees shown above.
	Post Masters Certificate students pay the above tuition and fee amounts.
	The Summer 2007 OIT Technology, Building, Incidental and Health Service Fees will be determined from the 2007 OUS Summer Session Fee Book.
