Advising and Thesis Forms

The Advising and Thesis Forms should be submitted in the following order:

1. Master's Program Mentor/Advisor Assignment form

2. Request for Advisory (Thesis) Committee AND Committee Approval of Thesis Proposal (These forms should be submitted at least 4 weeks before the Request for Oral Exam.)

3. Request for Oral Examination AND Committee Approval of Oral Exam (These forms must be submitted at least 30 days prior to your defense. Please add a few days of lead time for our administrative staff to process your forms before submitting them to Graduate Studies on your behalf.)

Please DO NOT submit all of these forms at once. All forms must be typed.

The Committee Approval of Thesis Proposal and the Committee Approval of Oral Exam form are internal departmental forms. All the rest can be found on the Graduate Studies Forms Page.
