Chinese Brochure MediaSample POLST Paradigm VideosOregon's success in improving end-of-life care through the Physician Orders for Life-Sustaining Treatment program has resulted in a major effort to assist other states in launching their own POLST programs. As part of this effort, Oregon's POLST Task Force has created a brief video that provides an overview of the program. A second POLST video (11 mns) walks step by step through the September 2004 version of the Oregon POLST form. This video is designed for health professionals who will be engaging in conversations with patients and their families as they make specific decisions to have or limit each category of life-sustaining treatment. A hospice patient’s story illustrates the use of POLST traveling from home to hospital, to nursing home and back home with his wishes clearly followed at each step of his transfer. This video is particularly sensitive to those in the Spanish speaking community, addressing the important role of bilingual family members and the use of an interpreter, as well as how culture and spirituality can play an important role during end of life care. The story of Jose, a man with advanced cancer, illustrates how Physician Order for Life Sustaining Treatment (POLST) is intended to work in multiple care settings.
