The POLST order form is for Health Care Professionals and organizations only. Individuals seeking to obtain a POLST form may do so through their physician.

The Oregon POLST form is updated approximately every two years. The most current version is June 2011. The next version of POLST is scheduled to be released in June 2013. All previous versions remain valid.
