Developing a Program

Sharing Information About Your Developing Program

Sharing information about your program helps colleagues develop a similar program, improves task force understanding, and helps you gain insight from others.

To be considered a developing program, please complete the following:

  1. Downloading and completing the program description template (.doc) describing your POLST Paradigm Program, and
  2. Email the completed template to polst@ohsu.edu.

Programs may be asked to update their information periodically.
