Mike is the Director of Pharmacy Services
with responsibilities including management
of pharmacy information systems, drug information/drug
policy services, residency programs, billing,
and contracting. Mike is also the Director
of the PGY2 Specialty Residency in Pharmacy
Informatics and PGY2 Specialty Residency in
Health-System Pharmacy Administration. Outside
of OHSU, Mike serves as the Vice Chair of
the UHC Medication Use Informatics Technology
Committee and serves as member of the APhA
Policy Committee and ASHP SPPM Programming
Committee. His administrative interests include
effective use of technology in health care,
clinical pharmacy services, mentoring/leadership,
and medication safety.

Yen Pham

BS, Pharmacy, University of Houston
Yen is an Assistant Director for Ambulatory
Services with responsibilities for all OHSU
ambulatory care pharmacy areas, with an
interest in expanding OHSU pharmacy services
into new areas such as specialty pharmacy
services, medication therapy management,
mail order and employee benefits. Her administrative
interests include ambulatory operations,
building consensus, and expansion of the
pharmacy profession.

University of Wisconsin-Madison
Jen is the Assistant Director for Central
Pharmacy operations with responsibility for
distribution, production and medication use
system workflow for inpatient care areas,
the Emergency Department and hospital based
clinics. Her administrative interests include
pharmacy operations and technology, teaching
and resident activities, the mentor/mentee
relationship and pharmacy association involvement
at both the state and national level.

PharmD, University of Washington
As the clinical operations manager, Joe has
responsibility for developing, expanding and
supporting all clinical activities within
the department. Joe maintains a clinical practice
in oncology, while representing the Department
on many multidisciplinary committees. His
administrative interests include improving
systems of care, patient safety, and clinical
pharmacy service development, while his research
interests are in supportive care for oncology
patients.

Overseeing the ambulatory pharmacy operations
at the Pavilion, Casey Eye Institute, Doernbecher
and Gabriel Park Pharmacies, Dan brings his
extensive community and ambulatory care setting
experiences to OHSU. Dan’s administrative
interests are in the fields of pharmacy automation,
patient safety and pharmacy policy, as a member
of the Oregon Health Resources Commission.

Justin is the Central Pharmacy operations
manager with responsibility for distribution,

and Pyxis Medstation® technology for inpatient
and outpatient care areas and the Emergency
Department. His administrative interests include
pharmacy operations and technology, teaching
and leadership development, pharmacy technician
advancement, and supply chain management.
