Clinic Referrals

Prior to scheduling an appointment the clinic must receive a completed medical referral form—Clinic Intake Form [pdf], which is different from an insurance referral—and recent, pertinent patient records from your primary care physician and/or neurologist.

Once your records are received, you will be called to schedule your appointment.

Clinic Visit

Required Clinic Forms: Prepare for your visit in advance by completing the following required forms and mail them to our offices at least 2 weeks before your appointment. If your appointment is sooner than 2 weeks, please either fax the forms to Neurology Clinic, Attention: Alma Kennedy at 503 418-9276 or bring the completed forms with you.

You may be frustrated by 15-minute appointments spacerelsewhere, but not here. As an academic institution, we have full hour consultation for new patients and 30-minute follow-up appointments. Allowing your provider more time to understand your problem, discuss your concerns, and explain treatment plans.

Our team trains physicians and other healthcare providers on how to care for PD and other movement disorders. We have a long history of research successes, always planning the next step to discover better treatments, causes, and cures.

TEAM approach

In addition to the best providers, you are accessing a team of individuals with advance training in Parkinson’s disease and neurological care. We believe in comprehensive or total team care for our patients, and our team meets regularly to optimize patient care.

  • Specialists—trained experts in diagnosing and treating PD and other movement disorders.
  • Nurses—one nurse will manage your overall PD patient care and the other nurse is an expert in DBS (deep brain stimulation).
  • Social worker—our Family Care Specialists works with patients and care partners to meet the challenges of the progressive nature of PD.
  • Therapists—all specially trained in the idiosyncrasies of PD and how best to manage it. We have two speech language pathologists, two physical therapists, and an occupational therapist.
  • Psychiatrist—Specialized assessment for neuropsychological issues.
  • Alternative therapies—We are fortunate to have a complimentary medicine team for consultation and care. This team includes a naturopath, acupuncturist, chiropractor, psychologist, and spiritualist.
