Session Goal: To introduce elements of the basic cardiovascular exam.

To establish/reinforce students' understanding that the PE group can augment the preceptorship experience.

 
 
Student Objectives:
Demonstrate appropriate methods for gown and drape for heart exam.

Distinguish between the carotid pulse and the jugular venous pulse.

Describe thenormal jugular venous pressure (JVP) and what an elevation means.

Demonstrate the measurement of the jugular venous pressure.

Describe normal location and size of point of maximal impulse (PMI) and what causes it.

Describe a cardiac thrill.

Describe the location of the following areas: aortic, pulmonic, base, apex, mitral and tricuspid.

Describe what is meant by "systole" and "diastole."

Distinguish between S1 and S2 and descirbe what creates S1 and S2.

Describe the cause of a physiologically split S2.

Describe what a murmur is and the characteristics that are used to describe one.

Describe what a gallop is and distinguish between S3 and S4.

Identify and be able to describe locations of radial, brachial, carotid, femoral, popliteal, dorsal pedal and posterior tibial pulses.

Describe and identify the midsternal, mid clavicular, anterior axillary, scapular and vertebral line, costal margin and suprasternal notch.

Identify the angle of Louis (the Manubrial-sternal junction and the structures corresponding to this level).

Demonstrate proficiency in Heart Exam using DVD checklist.

Demonstrate proficiency in Pulses Exam using DVD checklist.

 
Small Group Activities:
  1. Review student experiences in their preceptorships with the heart exam.


  2. Using the checklist, have students practice heart and pulses exams in pairs.


  3. Have students demonstrate the heart and pulses exams to their PE Tutor.


  4. In the last 15 minutes, students will break into 2 groups for a GOSCE-type exercise on the Cardiovascular exam.

Students should preview the Lung Exam lecture and the "Lungs and Thorax" exam on the physical exam DVD on PCM Online prior to today's session.
