CYTOPATHOLOGY ROTATIONS

GOALS AND OBJECTIVES:
Patient Care
Be able to perform fine needle aspiration biopsies (FNA) of lesions and perform adequacies under ultrasound or CT-scan guidance.
Medical Knowledge
Know the main laboratory preparatory techniques/procedures relating to processing and staining GYN and non-GYN specimens.
Understand the use of special stains in cytopathology, including immunostains.
Know the spectrum of normal and dysplastic cervical cytology.
Know the signs of inflammation and infection in cytologic specimens.
Know the spectrum of normal, reactive and neoplastic changes in body fluids.
Know the spectrum of normal, reactive and neoplastic cytology in FNA.
Practice-Based Learning and Improvement
Understand the process and significance of cyto-histologic correlation for GYN and non-GYN specimens.
Understand and learn from QA/QC procedures (screening negatives, resolving technical problems).
Interpersonal and Communication Skills
Be able to clearly convey assessment of specimen adequacy at the time of fine needle aspiration biopsy.
Be able to clearly convey cytologic interpretations in writing and verbally.
Professionalism
Professional interaction with all the cytopathology staff and clinicians is demanded.
Comply with HIPAA standards with regard to confidential patient information.
Systems-Based Practice
Know the administrative structure of the Cytopathology Department, and the role of each member, including the clerical staff, the laboratory staff, the cytotechnologists, and the laboratory supervisor.
Participate in consultative services performing fine needle aspirations, attending Tumor Boards to promote integrated healthcare delivery.
Description of Responsibilities by Rotation:
1st Rotation:
Goals:
Acquire basic understanding of laboratory procedures as well as the rules and regulations related to the cytopathology laboratory.
Begin to acquire screening and cytodiagnostic skills.
Introduction to the role of the FNA service.
Learning the role of the cytopathologist in optimizing patient care with specimen assessment and triage.
Encourage the importance of current cytopathology literature review to reinforce the concept of life-longlearning.
Objectives:
Note: The first month of cytopathology is primarily dedicated to reading, study sets, and pap smears with an unknown examination administered at the end of the month (SurePath pap test). Residents are responsible to review 3-5 unknown pap smears per day and attend all FNAs and all possibly adequacy assessments. They are also required to attend daily sign-out.
1st Week:
Learn about acceptance, accessioning and preparation of specimens for
processing.
Processing and staining of specimens with the Papanicolaou stain.
Preparation of specimens for screening.
Screening of specimens and assignment of cytotechnologist's provisional interpretation.
Residents are to screen at least 3 cervical-vaginal smears per day before the
cytotechnologist and the have those slides re-screened by cytotechnologist.

2nd Week:
Continue daily pap smear screening.
Review Cytopathology Study Set - Cervical-vaginal (GYN) smears found on 2nd floor DH
Read the following chapters, book and/or articles:
Manual of Cytotechnology (ASCP Press) - Chapter 28: Organization and Inspection
Review the College of American Pathologist (CAP) Inspection checklist for cytopathology lab
Read at least one article on cervical-vaginal cytopathology from the latest issue of ONE of the following:
Acta Cytologica
Diagnostic Cytopathology
Analytical and Quantitative Cytology and Histology
3rd Week:
Continue daily pap smear screening
Review thyroid cytology study set
Demonstrate to the satisfaction of the service director and/or Chief Cytotechnologist, proficiency with:
Diff-Quick stain.
Rapid H&E with touch prep material
Attend Thursday afternoon Breast Clinic with fellow/staff on FNA service
4th Week:
Continue daily pap smear screening
Review Cytopathology Study Set - Body Fluids
Introduction to Cytopathology FNA service:
-practice FNA on model with fellow/staff on FNA service
-perform at least 3 FNAs on patients during first month
Examination:
SurePath Pap Test administered by Chief Cytotechnologist
2nd and 3rd Rotations
Goals:
Acquire effective communication skills and information exchange with patients, clinicians and other professional associates
Increase medical knowledge
Continuous literature review to acquire investigatory and analytic skills
Residents are encouraged to consider research opportunities in cytopathology
Increased experience and proficiency in cytodiagnosis
Understanding of the ancillary testing used in cytopathology, including flow cytometry and specimen culture
Gain competency, proficiency and confidence in performing fine needle aspirations
Provide patient care that is professional, compassionate, appropriate and effective
Gather essential and accurate information about their patients
Apply medical experience and knowledge to direct appropriate specimen triage
Objectives:
Selected chapters in the Manual of Cytotechnology (ASCP Press).
Recommended reading includes Part 3: Nongynecologic cytology-chapters 15-16,18-19 and Part 4: FNA Cytology-chapters 21,22 and 24
Read at least one article from each of the latest issues of the following:
Acta Cytologica
Diagnostic Cytopathology
Analytical and Quantitative Cytology and Histology
Review cytology study sets especially focusing on body fluids and FNA of thyroid, breast and salivary gland
Continue daily review of 3-5 pap smears per day.
Responsibilities:
FNA service coverage
Primary cytopathology contact physician for clinician questions
Attend all cytopathology fine needle aspirations and assessment procedures
Note: All resident conferences are to be attended and the staff pathologist is to cover service during these lectures
Follow-up on ancillary testing results for fine needle aspiration cases
Participate in cytopathology sign-out with staff pathologist
Review cases prior to sign-out with staff pathologist to increase proficiency in cytodiagnosis
Supervision and Evaluation:
Residents are supervised and evaluated by the cytotechnologists, the cytopathology fellow and the staff pathologists on a daily basis
The chief cytotechnologist and laboratory director monitors completion of the 1st Month weekly objectives
Residents are evaluated formally at the end of each one-month rotation
