WORKING FORMULATION FOR NON-HODGKIN'S LYMPHOMAS

A working formulation for non-Hodgkin's lymphoma for clinical use - January 1980.

LOW GRADE LYMPHOMAS

Small lymphocytic (well differentiated lymphocytic) Follicular, mixed small cleaved and large cell (nodular mixed lymphocytic-histiocytic)

INTERMEDIATE GRADE LYMPHOMAS

Follicular, large cell (nodular histiocytic)
Diffuse, small cleaved cell (diffuse poorly differentiated lymphocytic)
Diffuse, mixed, small and large cell
Diffuse, large cell (diffuse histiocytic)

HIGH GRADE LYMPHOMAS

Diffuse, mixed, lymphocytic-histiocytic
Diffuse large cell immunoblastic (diffuse, histiocytic)
Lymphoblastic (lymphoblastic)
Diffuse, small non-cleaved cell (Burkitt's and non-Burkitt's)
Diffuse, undifferentiated, Burkitt's and non-Burkitt's

MISCELLANEOUS

Mycosis Fungoides/Sezary's Syndrome
Unclassifiable

ENDOMETRIAL DATING

16th Day
Subnuclear cabuoles
Pseudostratification
Mitoses, glands and stroma

17th Day
More or less orderly row of nuclei
Cytoplasm above nuclei and subnuclear cauoles below
Gland and stromal mitoses
Very, very, very minimal secretion

18th Day
Vacuoles above and below nuclei
Improved linear arrangement of nuclei
Gland mitoses rare
Stromal mitoses rare
Bubbles of secretion seen in luminal border

19th Day
A few vacuoles remain in cell. Mainly active evacuation
with intraluminal secretion
No gland or stromal mitoses
May look like day 16 but NO pseudostratification

20th Day
Peak secretion with "ragged" luminal border
Vacuoles are rare - all subnuclear vacuoles gone.
Inspissation may be beginning

21st Day
Abrupt onset of stromal edema
Gland secretion prominent (inspissated)
"Naked" stromal nuclei begin to appear

22nd Day
Peak edema
Marked appearance of "naked" stromal nuclei. Stromal cells small and dense
filamentous cytoplasm
Active secretion, but subsiding
Rare stromal mitoses

23rd Day
Prominent spiral arterioles
Periarteriolar cuffing with *enlargement of stromal cell nuclei and cytoplasm.
*(earliest predecidual change)
Stromal mitoses
Glands with secretory "exhaustion" - low columnar cells, luminal edges ragged

24th Day
Definite predecidual cells around arterioles with early subepithelial changes
Greater stromal mitoses
Ragged cell borders, i.e. secretorily exhausted

25th Day
Definite subcapsular predecidual
Inspissated secretion noted to begin
Early stromal infiltration with lymphocytes and occasional PMN's

26th Day
Generalized decidual reaction - decidual islands in stroma
Polymorphonuclear leukocytic invasion (lymphocytes may accompany or precede)

27th Day
Solid sheet of decidual
Marked leukocytic infiltrate. Polys prominent
Cells may show secretory exhaustion or may show active secretion
Beginning stromal clumping and fragmentation of glands

Menstruating
Disruption of capsular layer
Stromal clumping
Glandular break-up and hemorrhage
Variable leukocytic infiltration
Edema
After 24 hours, metaplastic alterations on surface
