Who is Eligible?
Anyone living with HIV/AIDS in Multnomah, Clackamas, Washington or Columbia Counties is eligible to become a Partnership Project client. For questions or scheduling, please call the Partnership/CAP Joint Intake Team at 503-517-3590.

How to Become a Client
Partnership Project partners with Cascade AIDS Project to give clients the convenience of doing one intake that applies to both agencies. Partnership Project staff members are located at the Cascade AIDS Project office in downtown Portland to provide telephone information on case management services as well as conduct intakes. Services
We provide information and referral services, as well as Intake, Assessment and Goal Planning. Clients are assigned to one of two tiers of service based on needs identified through the assessment. Through regular assessments by case management staff, clients may be moved between these levels of service as their needs change. We help clients connect with health care providers, including physicians, nurses, pharmacists, as well as mental health and substance abuse providers. We coordinate health insurance, ensure access to medical care, evaluate housing options, as well as an array of other client needs. For clients assessed to need Nurse Case Management, our Nurse Case Manager's monitor clients' medication and health and assist with coordination of medical care. Clients can leave services when their goals have been accomplished and can also return to services if needed.
Services are provided in a culturally and linguistically appropriate manner. We have five staff members who are fluent in Spanish. We also contract with the Immigrant and Refugee Community Organization (IRCO) to provide interpreters for clients fluent in languages other than Spanish.
Our service locations: Legacy Emanuel Hospital; Good Samaritan Hospital; Multnomah County HIV Clinic; OHSU; Cascade AIDS Project (intake, assessment and goal planning); and the Partnership Project office, located in Portland's Sellwood neighborhood.

Service Access
Service Access is designed to meet the needs of people living with HIV/AIDS and their families who need information, referral, short term goal planning, insurance and benefits assistance, help in accessing necessary medications, and brief crisis intervention.
Services are provided by Service Access Specialists.

Case Management
Case management service is designed to meet the needs of people living with HIV/AIDS and their families who need intensive, ongoing and regular contact to coordinate needed services. Clients receive long-term care coordination in relation to a variety of jointly identified issues such as housing, insurance, mental health, substance abuse, and non-HIV related health problems.
Services provided by Social Work Case Managers and Nurse Case Managers.

Nurse Case Management
Nurse case management is designed to meet the needs of clients who are experiencing the following: recent or current hospitalization; medical adherence issues; significant changes in health status or ability to manage activities of daily living; pediatric clients; multiple health and mental health diagnosis and end-of-life needs.
Nurse Case Managers are assigned to a health system or group of health systems to facilitate the best coordination with case managers, clinic nurses and medical providers.
All clients who receive an intake are assessed for their need for Nursing Case Management. Clients can also be referred by clinic nurses, case management staff, and medical providers.
