ID Badge/Access Card:
If you have lost your ID badge, there is a $5.00 fee that must be paid first at the Parking Office. Cash, checks and credit card is accepted. If your access card needs to be replaced, there is a additional $15.00 fee. You may want to ask your department if they will pay for the access card. There is a seperate charge if you are replacing a TriMet or C-Tran sticker.
Please be sure that all of the paper work for your new ID badge and/or access card is filled out completely. You will not be able to replace your items with uncompleted paperwork.

Process for obtaining ID Badge
Keys:
If your department has ordered keys for you to use, you will be notified via email or campus mail that your key(s) are ready to be picked up. Please bring your OHSU ID badge with you. We cannot allow the release any key(s) without an current OHSU ID badge. *You may designate a person from your office to pick up your keys.
*To designate a coworker to pick up your building key(s), you will have to write a note of permission and give the designee your ID badge to bring to the office. That person will also need their OHSU ID badge for the key(s) to be released.
