Medical Providers

All providers across the state of Oregon are encouraged to make referrals to our

community-based services. If you are the provider for a child or youth with special health needs who requires follow up for resources or care coordination in their local community please take a look at our CaCoon Public Health Nurse program as well as our Community Connections Networks multidiscipline teams.

We have some financial support available for families through our Family Support Program.  You may be able to help a family find money for something they did not think they would be able to afford.

If you are looking for information on child development we have some resources you may find helpful.

We offer a variety of Continuing Medical Education opportunities in the Portland area and around the state.  We encourage you to look into these opportunities to receive more training on issues of care for children with special health needs.
