Medical Home
The Oregon Medical Home Project supports primary care providers in their offices to improve their ability to better meet the needs of families who have children with special health needs. Project staff work with pediatricians, general practitioners, physician assistants, nurse practitioners and key office staff to help them
- build partnerships with families,
- recognize that the family knows what is best for their child,
- identify children within their practice with special health needs,
- organize their practice in a way that makes it easier to meet the needs of the family and child,
- coordinate medical care with specialists, and
- connect with community resources.
The medical home project is working actively with six model medical practices throughout Oregon, Childhood Health Associates of Salem, Mid Valley Children's Clinic of Albany, Treasure Valley Pediatric Clinic of Ontario, Southern Oregon Pediatrics of Medford, and OHSU Gabriel Park Family Practice Clinic of SW Portland.

the medical home approach, national, state and local resource lists and
suggestions for improving care coordination among medical practices and
community providers and other specialists.
