Transition to Adulthood
How we address this outcome.
The CDRC/Oregon Department of Education Interagency Team (COIT) meets quarterly to address the transition issues of adolescents and young adults who are served by both agencies. An Adolescent Transition Summit was held on May 2, 2003 where teams from around the state began the development of plans to address transition at the community level.
A continuation of this summit's efforts is the Youth Transition Learning Collaborative where six community teams from around the state are engaged in a year long process to support the development of local transition resources and systems. Training and technical assistance for CaCoon nurses, school nurses, and transition specialists in those six communities will focus on health aspects of youth transition.

Youth Transition Learning Collaborative
For our Youth Transition Learning Collaborative, we have communicated with system leaders at the state level and formed a faculty of national and local leaders, provided written and verbal information about promising practices and instructed teams in a improvement model tested by IHI. We have recruited teams of providers most interested in successful transition and families who want to see a difference.
