The Family's Role

We address this outcome with:

Our Family Involvement Network (FIN) program which is an integral part of our office. The paid FIN parents on our staff guide us and provide a constant family perspective to keep a family focus in everything that we do.

Our FIN program is working to develop family liaisons to sit on all of our community-based Multidisciplinary teams and partner in team decision making at the community level.

Our Medical Home resource teams include family members available to help other families and the staff in the primary care offices locate the information and resources that families need. The resource teams also work with primary care office staff to change the way services are provided in the office such as identifying ways to regularly request the ideas and recommendations of families.
