Raising a child with special needs can pose some challenges for your family. You and your family may need the services of many different professionals and agencies. You may have to travel away from home for specialized health care. You may need information about available services for your child.
CaCoon can help!
What is the CaCoon Program?
CaCoon is a public health nurse home visiting program for families who have children with special health needs. This program provides support for families in your community by:
- Helping parents find resources
- Helping coordinate services
Because you and other family members have a central role in the care of your child, all services are planned around the desires and concerns of your family.
How Can CaCoon Help?
Through CaCoon, you can receive help from a local public health nurse, who can:
- Answer questions about your child’s health needs and special care
- Help you find special services as close to your home as possible
- Help you prevent problems that could be related to your child’s health condition
- Support you in times of stress and crisis
- Work with your primary care physician to coordinate health care and specialty services
- Provide information about other services and programs your child may qualify for
Who Can Use the CaCoon Program ?
Families with children and young adults, from birth through age 20, who have chronic health conditions or disabilities. CaCoon services are available to all families regardless of income.
